Medical Records, Privacy and Your Rights
At Spencer Road Family Practice confidential records will be kept of your visits and treatment. The content of such visits will not be divulged without your written consent or where required by law. Patients are entitled to see their medical records, which can be done during an appointment with your regular doctor.
If you wish to transfer your records from a previous surgery, you will be required to complete a Transfer of Notes form.
If you require your records sent from our practice to another location, you will need to complete the Transfer of Notes form at the new location and they will fax us the consent form, as all record requests must be made in writing.
Here at Spencer Road Family Practice we comply with the “Privacy Act 1988 (Cth)” . This includes the way we collect, store, use and discloses health information.
Our practice encourages you to fill out a Patient Feedback Form and place in the Feedback Box. If you are concerned with any aspect of the service we encourage you to let us know. We are keen to hear your feedback whether negative or positive.
We believe problems are best dealt with within the practice. We welcome you to speak with the Practice Manager to discuss your concerns. However, if you feel you need to discuss outside the practice, you may contact Health & Disability Services Complaints Office at GPO Box B61, PERTH 6838 or via their website www.hadsco.wa.gov.au
